Alcohol, Smoking and Substance Involvement Screening Test
A World Health Organization (WHO) screening tool that assesses the level of involvement with alcohol, tobacco, cannabis, cocaine, amphetamines, inhalants, sedatives, hallucinogens, opioids, and other drugs.
Answer each question honestly about your substance use. The ASSIST helps identify your risk level for each substance separately.

2.During the past three months, how often have you used the substance(s) you mentioned?
3.During the past three months, how often have you had a strong desire or urge to use?
4.During the past three months, how often have problems with your health, social, legal, or financial situation resulted from your substance use?
5.During the past three months, how often have you failed to do what was normally expected of you because of substance use?
6.How often have you been concerned that your substance use is out of control?
7.Have you ever used drugs by injection?
