What Is Gamma-Hydroxybutyrate (GHB) Addiction?

Gamma-hydroxybutyrate (GHB), also known as liquid ecstasy, is a central nervous system depressant that acts as both a GABA-B receptor agonist and binds to specific GHB receptors in the brain. It is produced endogenously in small quantities but is also synthesized as a chemical precursor (gamma-butyrolactone, or GBL) that is converted to GHB in the body. GHB was briefly marketed as a pharmaceutical for narcolepsy but was withdrawn due to abuse and danger. It remains a substance of significant abuse, particularly in party and club settings, for its euphoric and sexually disinhibiting effects at lower doses and its use as a date rape drug. GHB has an extremely narrow margin between a functional dose and a toxic dose (dosing window of only 1 to 2 grams), making overdose common and dangerous. Overdose can result in sudden loss of consciousness, respiratory depression, bradycardia (dangerously slow heart rate), seizures, and death. Chronic GHB abuse creates severe physiological and psychological dependence with an extraordinarily difficult withdrawal syndrome. Withdrawal & Detox

GHB withdrawal is severe and potentially life-threatening, requiring medical supervision. Withdrawal begins 6 to 24 hours after the last use and progresses rapidly. Early symptoms include anxiety, insomnia, tremors, and muscle pain. Acute withdrawal symptoms peak 12 to 24 hours after last use and include severe anxiety or panic, hallucinations, delirium, dangerous elevations in blood pressure (sometimes exceeding 200/110 mmHg), extreme tachycardia (heart rate sometimes exceeding 150 bpm), seizures, and sweating. The withdrawal syndrome can last 5 to 14 days and, in severe cases, can be medically dangerous. Unlike opioid withdrawal, which is extremely uncomfortable but rarely medically dangerous, GHB withdrawal can cause seizures, dangerous cardiovascular changes, and other medical emergencies. Some individuals experience protracted withdrawal with persistent insomnia, anxiety, and mood disturbance lasting weeks.

Treatment for GHB addiction requires medically supervised inpatient detoxification in virtually all cases. Outpatient detox is not safe due to the severity and potential dangerousness of withdrawal. Medical detox includes continuous monitoring, benzodiazepines (typically in escalating doses) to manage anxiety, tremors, and seizure risk, and careful monitoring of vital signs and cardiac function. Barbiturates or other sedatives may be needed for severe withdrawal. Once medically stabilized, clients transition to residential or PHP treatment that includes psychiatric assessment and care, CBT for addiction and co-occurring conditions, group therapy, family involvement, and aftercare planning with strong linkage. The psychological component of treatment addresses the underlying reasons for GHB use and builds healthier coping mechanisms.
